1. Student must be currently enrolled at WRUSD.  
2. Student must have current eyeglasses prescription.

4. Must sign for voucher in person. Bring a copy of your current prescription to the WRUSD District Administration Building located in Ft. Defiance, AZ.  Our office is located on the first floor in Room A-147.
